主旨：

本校欲申請國科會115年度專題研究計畫案之教師，如需送基因重組暨感染性生物材料實驗申請同意者，請於114年12月12日(星期五)前送交審查，請查照。

說明：

一、依國科會規定，本校教師欲向國科會申請專題研究計畫，凡涉及基因重組與感染性生物材料實驗者，需填具基因重組暨感染性生物材料實驗申請同意書，經本校生物安全會審核同意後上傳。

二、凡本校欲申請國科會115年度專題研究計畫之基因重組暨感染性生物材料實驗申請同意者，請檢具下列文件，於114年12月12日(星期五)前送交審查(陽明校區：線上申請；新竹校區：紙本申請)，逾時恕不受理。

三、如需申請或加辦申請「感染性生物材料實驗」同意者，請以本同意書申請方式辦理。

四、基因重組暨感染性生物材料實驗申請案並無提供送審中證明，提醒有時效限制之申請教師注意及配合。

五、生物安全會擬訂於114年12月下旬召開會議複審申請案，逾期申請之案件需俟審查會議結束後，於115年1月份再次開放受理審查。

Faculty members intending to apply for 2026 National Science and Technology Council's research project who need to apply consent form of using recombinant DNA or infectious biological materials, please submit the application documents for review before December 12.

Illustration:

In accordance with the regulations of the National Science and Technology Council, faculty members who wish to apply for a research project involving recombinant DNA and infectious biological material experiments must fill out and submit consent form of using recombinant DNA or infectious biological materials, which must be approved by the university's biosafety committee before uploading.

Faculty members who wish to apply for 2026 National Science and Technology Council 's research project involving the usage of recombinant DNA or infectious biological materials must submit online system (Yang Ming Campus) or one copy (Hsinchu Campus) of the following documents to the Environmental Protection and Safety and Health Center for review before December 12, 2025. Late submissions will not be accepted.

1.Consent form of using recombinant DNA or infectious biological materials.
2.The cover of National Science and Technology Council project proposal.
3.Abstract of proposal.
4.Experimental materials and methods.
5.Documents proving the level of biological risk of the materials.
(If the system has biological material data, no additional data is required.)

Those who need to apply or additionally apply for "infectious biological materials experiment" consent should proceed using this consent form.

Given that no proof of submission for review will be provided for the application cases of recombinant DNA and infectious biological material experiments, faculty members who would like to submit the consent form please pay attention to the application deadline and cooperate accordingly.

The biosafety committee will convene a review meeting in late December 2025. The applications submitted after the deadline will be reviewed until next review meeting in January 2026.
