校務研究資料庫申請與諮詢業務，需收集申請人相關個人資料。填寫表單前請先閱讀以下說明：</div>\r\n\r\n<div class=\"ed_list\">\r\n<ul>\r\n\t<li>收集目的：用於身份核對、申請審核、聯絡通知、系統權限開通及校務統計分析。</li>\r\n\t<li>欄位收集必要性說明：\r\n\t<ul>\r\n\t\t<li>基本資料（姓名、單位/職稱、人事代號/學號）：用於核對申請人身分與校內權限範疇（必填）。</li>\r\n\t\t<li>聯絡方式（電話、Email）：用於寄送審核結果、系統開通通知及業務聯繫（必填）。</li>\r\n\t\t<li>計畫/需求說明（研究計畫、資料需求）：作為審核資料使用範圍與權限發放之依據（必填）。</li>\r\n\t</ul>\r\n\t</li>\r\n\t<li>權利影響：台端得自由選擇是否提供上述個人資料，惟若未提供或提供不完整、不正確，將無法完成本中心相關服務之申請、審核及後續作業。</li>\r\n</ul>\r\n</div>\r\n</div><div
